On my Sidewinder 3D pro..  I have :
trigger in front
HAT does it's normal thing
left of the HAT is my secondary fire button (missles)
right of the HAT-top button "cycle primary"
right of the HAT-bottom button "Cycle secondary"
button "C" is the "Mines, counter measures"
and A,B, and D are audio taunts
I don't use the "throttle slide" or the "shift" buton (or the button w/ the
arrow on it....)
I use the keyboard for the Forward and Reverse..  (A and Z)
