I have visited the prison tower, Killorn Keep, and the ice world.  At each
place, I recovered the blackrock gems and I merged them with the larger gem
(the three faces turned blue).  But, I still cannot enter any of the other
faces of the gem.  What do I need to do?

The only thing that I really haven't done (that I know of) is go on Altara's
quest.  I could not figure out how to use the dagger to locate, get to, and 
kill the imp that she was talking about.

Did you have Nelson enchant the gems?
Also you must wait until the faces change color before entering.

You have to walk thru a hidden door in the lava wall.  Then use the
knife just as you would use a sword, or any other weapen.

Altara reveals that the imp is in your world, ie. Britannia.  He's a bit
tough to find.  He's below the castle, as she indicated, which only leaves
three levels where he could be.  Have you thoroughly explored those levels?
You might want to do so - there's lots of useful things there.  Nevertheless...

Look on the southeast corner of the fourth level.  You'll have to jump into the
water.  The current carries you to a little path, and at the top you'll find
an opening marked with lava.  Down in this area is where the imp is.  Kill him,
and you can continue.

Each gem you merge with the large gem opens up another facet of
the large gem... the first facet is opened for us already, so
if you have merged 3 warm blackrock gems you should have four
facets available to you...  somewhere in the fourth facet will
be the gem to open the next facet, etcetera.

Having the dagger Altara gives you opens up a spot that wasn't
there previously (on level 4, I believe in the northeast corner,
though that may be wrong)
