	Do you mean the 4th statue head?
The first statue head is in the n/e corner of the first level,
the "attack melon" level.  It is behind the fire.

The second is in the sewer level (with the "evil polyps" on the
ceiling).
The third is in the "skeleton courtyard" level (level 3?)
The fourth head is in the same level, right there in the moat
area.

...or do you mean the fourth shape, the frogthing?  You
will get the 4th shape right before you go on to level 7.

This leads to my question.  I played this game and got stuck
on level 7.   How do you go on?   I have no idea.

	Sorry for not clarifying the description.  Yes, I mean the
    statue head.

	I have already gotten the fourth shape... not face :) ... after
collecting four statue faces.

Well, that depends on which visit to level seven this is for you.  If it
is the first time thorough, then all you have to do is kill off all the
guys wearing cloaks, all the skeletons and destroy those discs floating in the
four rooms.  Then, just walk up the middle and you will see another one of
them nasty four armed death lords again!  Kill him and then you go into a room
that is sort of the hub of the game.  That's where all the teleports are to all
the other worlds.
