Watching very carefully for where deathclaws, mirelurks hide and roam and avoiding the areas entirely -- and skulking around. It's a much more dangerous world than fallout 3. Best stay up north at first because it gets more dangerous down south. I leveled up. After getting some of my main attributes up to par I focussed on sneak, action-boy/girl, and lockpicking. action-boy/girl is a nice perk since it makes VATS fill-up quick, so with enhanced non-auto gun perks I can take on an army. With deathclaws I reload to just before the encounter, look for an escape hatch of some sort so I can run and recuperate, then lay some mines. In some cases there's no escape hatch so you have to run, lay mines behind you, be quick on the stimpack hotkey. The suicider supermutants are the worst 'cause you have only the one chance to take them out. If they get you you're gone. It's a HUGE game. If you like this kind of game it's an incredible bang for the buck. They put a lot into the development of this game and it's a totally satisfying experience. (all my niggling complaints aside!)
