The requirement is to do Broken Shore once, and then it's unlocked for
all your characters (possibly faction-specific, I'm not 100% sure on
that one, certainly no need to do it more than twice) - the
Karazhan/Ulduar bit is retired as of August 30th, accessible via an
NPC for those who want to see it, but not part of the overall sequence
that is required in order to access Broken isles. 

Thought I'd find a source for this to make sure I'm remembering right:

character, Holgar Stormaxe will offer an option to skip it and
teleport straight to Dalaran in the Broken Isles, with [100] In the

Archivist Elysiana in the Violet Citadel in Dalaran."

Haven't got a source for the Kara bit being out of it entirely but I'm
sure I read that one as well.
