One of my shareware CDs, a collection of 1,500 add-on levels for Heretic, actually gives the name of the person who created the map. It's very helpful because you eventually learn who designs the best new levels and can load all of their maps at one time. This was not the WizardWorks collection but one from Laser Magic. That's where I got the assumption that many of these collections do credit the level designers and possibly ask their permission to use their levels.
