to tylko czesc kodu
ten kod znajduje sie w formularzu, ja uzywam ruby on rails ale 
potrzebuje zrobic to w javiescrypt wiec potrzebuje wyczyscic tylko te 
pola ktore znajduja sie w <div> o danych id na naczisniecie close czyli 
on click, ten czysczenie powinno sie znajdowac w tej czesci
