You want a major plot twist? get this 

THE GUARDIAN IS THE AVATAR.

If you keep the results of U8 we see that the avatar had to trash his 
virtures for power and the ability to leave Pagan, and in the end he gets 
even more power than he ever before.  Well after U2 and underworld 2 we 
see that time is an easy road to travel, so maybe U9 is the place where 
the avatar gets the chance to become his enemy, to gain the power that 
will turn him to evil, the coruption.

This is good.

The avatar has the power to destroy, the power to devastate the world, 
but he has to relearn his virtures, to learn that power is as important 
as purpose.  Lets redo ultima 4 in a ways, if the avatar uses his power 
to destroy or over uses it in any way he loses his virutue.(remember the 
skull of Mondain?)  It could be that The Power is not free in the sense 
that someone just gav the avatar a box of infinte energy, but rather the 
avatar is linked to the plane of Ether and if the avatar uses that power 
to much (ie kill a city, move mountains etc.) then the are consequences.  
Magic fails for others, emps die, healing magic is corupted, people go 
crazy ... Now the purpose of Accension is to rise above the coruption, to 
rise above the petty wars of power, to rise to humility.  U9 could be a 
true Ultima if there is a plot that explores the nature of virtue and the 
nature good and evil.  Make Ultima 9 more than a thinking game, make it a 
drama, make it a game that makes a person feel good for thae right reasons.

Plot ending for the above situation, when the avatar gives his power back 
to the ether and has shown that the Virtues are stronger than "titanic 
forces" then the avatar can't become the Guardian, and the Guardian will 
no longer exist.

You can explain how the Guardian exists in the past by sayin that the 
Avatar was able to become the Guardian in the past and it is the 
potential Guardian that creates the Guardian.  The Guardian is a 
potential being really wild mind games here.
