It might be extreme bad luck, as I've never noticed a similar pattern, the only thing I tend to notice is anything with a good reward seems to have about 20% chance to succeed, whatever the chace might say, while anything with a reward which is useless to me will always succeed, even when I throow totally unsuited followers at it lol. But probably just RNG in my case at least! I'll pay more attention to ones like yours, mine usually succeed i think, but haven't paid a huge amount of attention. I'd love to actually see a mission for some 645+ gear sometime though, haven't had one in about three weeks now.
