I think I remember PunkBuster being the cause of this behavior a long time ago when PB was first
included in Q3, but I really can't remember what we did to get around it before PB took care of
it with an update.  I'd try the commands to disable PB...something like:

seta cl_punkbuster "1"
seta cl_anonymous "0"
seta sv_strictAuth "1"
seta sv_punkbuster "0"
seta cl_allowDownload "1"

I'm not really sure which might be appropriate or if they even have anything to do with your
problem, but it's worth a shot.
