Hi Joseph,
I (and many others) have been having the same problem with the wheel mouse.
People have said shutting down Q3A and restarting it has cured it for them also installing the MS Intellimouse drivers seems to have worked for some people.
It seems to be ok when you are setting the controls up in the 'control' interface but, from my experience as soon as Q3A is started it don't work.
The way I cured it was to make sure I had everything in my q3config.cfg just as I wanted it (binds, skin, rate, max fps etc) and then set the file's attributes to read only.As Q3A tries to write to the cfg everytime you play it,you will see the message "Could'nt write to q3config.cfg".This is because it's 'read-only' but since doing it I have had my mousewheel and mouse3 button functioning without any hitch.
I'm not sure if this is the best way to do it but it sure as hell has worked for me.Remember you won't be able to change anything without clearing the read-only attribute first but after any changes if you set it back to read-only,you're binds will continue to work fine.
I hope this will help you out as I know how damned pissed I got with it changing back all the time.
Happy Fraggin,Piper.
