I tried lot's of different ways to solve this problem, but the best one 
I use always is to demolish one square direct in front of the train 
(usually the one with signal). Then build a depot for the lost train to 
get into. This keeps the rest of the PBS-block quite safe (although 
there were cases of trains getting an PBS-clearance for a route which 
_crossed_ the train getting into depot). When the lost train gets into 
depot it reverses and goes back (in most cases) to the right track.

I have played a lot with PBS. It is a great tool, but you have to use it 
with extreme care. As for me, it would be useful to switch the PBS-iness 
of a signal system as easily as the presignals. Often I get PBS status 
on crossings that i would like to keep "normal". Usually in such cases 
trains collide there :)
