I would guess that you got it from the company (forget which) that has coded
Adult Content to be off at all times. There are work arounds though.

Here's what Nutwrench found for me.

Patch instructions for the Australian version of Duke 3D
--------------------------------------------------------

This file details the steps to patch the Australian (Manaccom) version
of Duke 3D to allow the re-enabling of adult mode. (Censorship Sucks!!!)

This is a very quick and dirty fix and requires the use of the DOS debug
utility. I have chosen to distribute this patch in this form in the interest
of getting it out as soon as possible and I dont have the time to write
an automated process.

The steps to follow are:-

1. Backup your original DUKE3D.EXE file incase you make a mistake :)
    COPY DUKE3D.EXE DUKE3D.BAK

2. Rename the DUKE3D.EXE file to DUKE3D.BIN
    REN DUKE3D.EXE DUKE3D.BIN

3. Run the DOS debug program on the DUKE3D.BIN file
    DEBUG DUKE3D.BIN

   3a. Enter the following debug commands
        -E 200 58
        -E 210 58
        -W
        -Q

4. Rename the DUKE3D.BIN file back to DUKE3D.EXE
    REN DUKE3D.BIN DUKE3D.EXE

5. Edit the DUKE3D.CFG file
    EDIT DUKE3D.CFG

   5a. Look for the line containing...
          Environment = " "
        and change it to
          Environment = ""

   5b. Look for the line containing...
          Out = 1
        and change it to
          Out = 0

   5c. Save the file and exit back to DOS

That's it, you are now ready to play Duke3D in all its true glory.

Another way that a friend of mine found is to start duke3d, and just before
the first splash screen comes up, restart the computer. Upon restarting,
you should find a number of new files. Rename one of these to *.exe and run
it. When I saw this done, he hadn't done it for a long time, so I'm not
sure if you need do exactly what he did, which was to rename a file as I
said, try and run it, it didn't work, rename a different file, which did
work, and turn the parental lock off from the .ini file. If you do it this
way, you may only need to look at the file sizes, and rename the new file
that is closest to the original *.exe file.
