Well it happened to me today...

I had found a Ring of Engagement from either the Skeleton King or
Oozedrool and in another game I was playing in one of the guys died and
droped all his stuff.  We got the message "Duplicate Item Detected" and
so when I res'd him and he got his stuff he said he was not missing
anything... So I checked my inventory and sure enough the Ring of
Engagement was missing.  So I asked if he had one of these too and he
said he did not!

Conclusion:  Patch was good at ridding us of TK but sucks overall!!!

That's BS..the patch will only delete things if it's dropped, not in
inventory.  Unless it's a rare-bug, I doubt it..

I'd say the rare bug exists, then.  A friend lost a royal circlet over the
weekend.  In addition to not being duped/cloned, it was the only royal
circlet in the game.  When another player was killed, he got the dupe
warning, and the circlet was gone.

All the players were in my living room, so I know no one else had a
circlet.
