I got a "fifth page" when I stepped on a shrine and got a message "magic
is not always what it seems to be". This reduces one spell by one level
and increases all of the others but also  created a "fifth page" which
appeared each time I opened the spell book until I returned to town by
portal. After that the spell book returned to normal.
