This happened to a friend of mine, several years ago:

A long evening of hacking on his Atari 800 video game ended on a
sour note when his program crashed by jumping to the disk-format
routine in the OS ROM.  You can imagine his puzzled expression, soon
changing into enlightenment, when the drive, without warning, began
that distinctive "tickticktick..."
