Tops-10 7.xx as well as TOPS-20 4.x onwards used extended addressing.  The
GALAXY subsystem (unrelated to VAX clustering) also adopted extended addressing
so that's pretty much all of the operating system.

Fortran v11 (and possibly v10 and v7, I don't remember for certain) used
extended addressing.
